Think of your pain like a loud smoke alarm going off in your house. A cortisone injection is like taking the batteries out of the smoke alarm. The noise stops, and you feel immediate relief, but the fire that triggered the alarm is still burning. And while you’re enjoying the silence, the fire may actually be getting worse!

PRP takes a different approach. Instead of silencing the alarm, PRP calls in the fire department. It recruits your body’s own repair cells, delivers growth factors to the damaged tissue, and works to actually put out the fire. The alarm might keep ringing for a little while during the process (PRP relief is gradual, not instant) but when the fire is out, the alarm stops on its own because there’s nothing left to detect.

Cortisone is a powerful anti-inflammatory that can provide dramatic pain relief within days, but it doesn’t repair tissue. Once the effect wears off (typically 6 to 12 weeks), the underlying problem remains. PRP works more slowly, stimulating actual tissue repair over weeks to months, but the results tend to last much longer.

Another part of the analogy that really matters: taking the batteries out of the smoke alarm repeatedly can cause real damage. A landmark 2-year trial found that patients receiving cortisone injections every 3 months had significantly greater cartilage loss on MRI without any improvement in pain. Repeated cortisone can also weaken tendons and thin skin at the injection site. PRP, by contrast, has an excellent safety profile with no significant adverse events beyond temporary soreness.

PRP vs. Physical Therapy

Don’t look at it as PRP versus physical therapy, they work together synergistically. Physical therapy is the foundation of treatment for nearly every musculoskeletal condition, but the combination of PT with PRP appears to enhance outcomes. A 2026 meta-analysis found that PRP plus exercise reduced pain significantly more than either treatment alone, exceeding the threshold for what patients can actually feel. For tennis elbow, a 2-year trial found PRP produced greater improvement than physiotherapy alone, and patients who received PRP had a 0% surgical rate compared to 20% in the physical therapy group.

PRP vs. Surgery

PRP is generally not a replacement for surgery when surgery is clearly needed. Complete tendon tears, bone-on-bone arthritis, or mechanical problems like locked meniscal tears typically require surgical correction, but PRP can play two important roles relative to surgery:

  • Delaying or avoiding surgery. A large retrospective study found that PRP delayed the need for total knee replacement by a median of 5.3 years, with nearly 86% of patients avoiding surgery during a 5-year follow-up. For patients with mild to moderate disease, PRP can meaningfully extend the window of non-surgical management.
  • Enhancing surgical outcomes. When applied during rotator cuff repair, PRP reduced retear rates by 62% in a meta-analysis of 7 randomized trials, with improvements in pain and function. Even when surgery is necessary, PRP can improve the quality of healing.

The Treatment Ladder

Rather than competing options, think of these as steps:

  1. Conservative care – activity modification, weight management, bracing, over-the-counter pain relief
  2. Physical therapy – essential non-surgical treatment
  3. Injection therapy – cortisone for acute flares; PRP for longer-term repair, ideally combined with continued exercise therapy
  4. Surgery – when non-surgical options have been exhausted or the condition clearly requires it

The goal is to use the least invasive, most effective treatment for your situation and to move up the ladder only when needed. For many patients, PRP occupies a valuable middle ground: more targeted than physical therapy alone, less invasive than surgery, and aimed at genuine tissue repair rather than just symptom management.
